چکیده
OBJECTIVE To study the effect of radiological quality of surgical reduction on the clinical outcome of the intra-articular fractures of the distal humerus. MATERIAL AND METHOD Ninety seven intra-articular fractures of distal humerus were treated at our department by open reduction and internal fixation, using a trans-olecranon approach. Patients were assessed as per the scoring system of Caja and Moorani et al. At final follow-up, radiological and clinical parameters were evaluated to assess the effect of quality of surgical reduction on the clinical outcome of the patient. RESULT The mean radiological score was 15 points. Patients whose radiological score was 20 had excellent clinical result. Fractures which failed in one criteria, radiological score 15, had good to excellent clinical result. Fractures which failed in two criteria (score 10) failed to show any excellent clinical result. Correlation between radiological and clinical scores was statistically significant (P= 0.0000). CONCLUSION Severity of fracture affected radiological, functional and total score. Patients with higher radiological score had better functional outcome.

Background and Aim: Intra-articular fractures of distal humerus in adults present a controversial challenge for orthopedic surgeons. Minor irregularities of the joint surface of the elbow usually cause some loss of function. This can usually be minimized by early, accurate open reduction with sufficiently rigid fixation to permit immediate motion. The posterior approach combined with Olecra...
متن کاملDISPLACED- COMMINUTED INTRAARTICULAR FRACTURES OF THE ADULT DISTAL HUMERUS: FUNCTIONAL RESULTS AFTER INTERNAL FIXATION
Background: Intra-articular T- type or Y- type fractures of the distal end of the humerus are relatively rare, and consequently few surgeons will be able to attain extensive experience with the surgical treatment of these injuries. The current emphasis is on operative intervention with rigid internal fixation and early mobilization. Methods: In this prospective and retrospective study, we r...
متن کاملCorrective Osteotomy for Intra-Articular Distal Humerus Malunion
Background: An intra-articular distal humerus malunion can be disabling. To improve function, reduce pain and/ or prevent further secondary osteoarthritis an intra-articular corrective osteotomy can be considered. Herein we present the indications, practical guidelines for pre- operative planning and surgical technique. Subsequently, we provide long-term results in a small series. Meth...
متن کاملA Pitfall in Fixation of Distal Humeral Fractures with Pre-Contoured Locking Compression Plate
Anatomically precontoured locking plates are intended to facilitate the fixation of articular fractures and particularly those associated with osteoporosis. Fractures of the distal humerus are relatively uncommon injuries where operative intervention can be exceptionally challenging. The distal humeral trochlea provides a very narrow anatomical window through which to pass a fixed-angle locking...
متن کاملInternal plate fixation versus plaster in displaced complete articular distal radius fractures, a randomised controlled trial
BACKGROUND Of all distal radius fractures, 25 % are complete articular fractures (AO/OTA type C fractures). Two thirds of those fractures are displaced and require reduction. According to several International Guidelines, adequately reduced intra-articular distal radius fractures are best treated non-operatively with plaster immobilisation, while surgical fixation is suggested only when the art...
